What's actually at stake.
Safety instrumented systems exist for one reason: to stop people getting hurt when something goes wrong on a hazardous plant. A design error nobody catches doesn't stay a design error.
IEC 61508 and IEC 61511 both require a functional safety management system to be in place before lifecycle work starts. Most contractors assume it's covered somewhere in their QMS. Usually it isn't, and the gap shows up at the worst possible moment: at design review, during an FSA, or when a client asks to see it.
Catch a mistake at concept and it's a line on a worksheet. Catch it at site and it's redesign, re-verification, reissued documents, site labour and a late handover. Miss it completely and under UK health and safety law the person who signed it off can be prosecuted, not just the company.

Every contractor I speak to knows they need one. Almost none have one. Not because they can't, but because the engineer who'd build it is the same engineer keeping three live projects on the road. "We'll sort the FSMS after this shutdown" becomes a sentence you've said for two years.
I used to sell templates to fix the cost problem. I sold a lot of them. Then I rang round to see how people were getting on, and the honest answer was that most were still sitting in a SharePoint folder. The cost was never the real blocker. Time was, and so was proof that the thing was worth having once it existed.
